If your plan is not listed here please check with your insurance carrier to determine whether the provider you wish to see at Lakes Counseling Services is in your network. Lakes Counseling Services also offers a sliding fee scale for individuals interested in paying out-of-pocket for services.
Co-Pays
Insurance requires that co-pays are due at time of service.
Monthly Statements
Statements will be mailed to clients on a monthly basis and will provide an accounting of all monies paid, a record of all insurance submissions, and any remaining balances.
